Tadalafil 20 mg and Dapoxetine 60 mg Tablets- It is a prescribed combination of tablets, which are prescribed to treat ED and PE in men. This medication is a phosphodiesterase type-5 (PDE-5) inhibitor and selective serotonin reuptake inhibitor (dapoxetine) and thus falls into two groups of pharmacological drugs. 

This medicine is primarily consumed by men to enhance the quality of sexual performance. It assists in achieving and maintaining a rigid erection in addition to controlling premature ejaculation. Henceforth, it can increase self-assurance.

How Tadalafil and Dapoxetine Tablets Work (Mechanism of Action)

Tadalafil 20 mg, by its action on the enzyme PDE-5, increases the intracavernosal concentration of cGMP, hence resulting in the vasodilation of vessels, increasing the blood flow into the corpora cavernosa when excited, and ultimately producing an erection.

Dapoxetine 60 mg is a short-acting SSRI. It inhibits the serotonin reuptake from the synaptic cleft and hence increases the serotonergic transmission to the postsynaptic receptor, resulting in the delay of the ejaculatory reflex and prolonging the intravaginal ejaculatory latency time (IELT), hence achieving control over ejaculation.

Medical Uses of Tadalafil-Dapoxetine Combination Tablets

A long-acting performance tablet for males is prescribed for

1. Erectile Dysfunction (ED)- it is characterized by failure to achieve or maintain a penile erection sufficient for satisfactory intercourse.
2. Premature Ejaculation (PE)-it is characterized by ejaculation during or immediately following penetration with minimal sexual stimulation before the male wishes it.
3. Comorbid ED and PE – Both ED and PE are found to coexist in adult males.

It is prescribed for adult males only. It is not a cure for ED and PE; however, it aids in ED and PE when taken on doctor’s suggestion.

Dosage and How to Take ED and premature ejaculation medicine

Take the medicine as directed by your physician. Never treat yourself or change the dosage without the advice of your doctor.

  • Dosage: Take one tablet (Tadalafil 20mg + Dapoxetine 60mg) orally with water.
  • Timing: take 1-3 hours prior to expected sexual activity.
  • Take with or without food: it will not affect how well this medicine works. However, it is important to note that if you take this medication after eating a high-fat meal, onset may be delayed.
  • Do not crush or chew the tablet: it should be swallowed whole.
  • Frequency: only once in 24 hours.
  • Onset and duration of effects: Tadalafil effect may last up to 36 hours and Dapoxetine effect may last between 1-3 hours after administration.

How Long Does It Take to Work?

Tadish-D has a onset of work from 30-60 minutes after being taken orally. The effects of both Tadalafil and Dapoxetine together may result in an effective window for sexual activity of 4-6 hours. Tadalafil has a longer half-life (around 17.5 hours)

Common Side Effects of Tadalafil-Dapoxetine Tablets

The following side effects may occur in some patients. They are usually mild and resolve on their own:

  • Headache
  • Nausea
  • Dizziness or lightheadedness
  • Facial flushing (warmth/redness in face, ears, or neck)

Seek immediate medical attention if you experience a prolonged erection lasting more than 4 hours (priapism), sudden vision or hearing loss, severe chest pain, or fainting.

Warnings and Contraindications

Do not take this medication if you:

    •  Have suffered a heart attack in the last 3 months, a stroke or cardiac failure within the last 6 months,
    •  Are taking nitrates (medication for chest pain) or nitric oxide donors in whatever form, as combined use can result in a severe drop in blood pressure.
    •  Have severe impairment of the liver or end-stage kidney disease or are less than 18 years of age,
    •  Are female and the drug is exclusively intended for adult males,
    •  Are sensitive to tadalafil or dapoxetine or to any other component of the medication,
    •  Have low blood pressure (hypotension), retinitis pigmentosa, or a bleeding disorder or be taking medication that can block CyP3A4 enzymes (for example, ketoconazole or ritonavir).

Precautions While Taking ED and Premature Ejaculation Medicine

The following are necessary precautions that you should take while consuming this medication.

  • You should not drive or use machinery for a number of hours after taking the tablet, as it can make you dizzy.
  • You should not eat grapefruit or drink grapefruit juice. The effect this can have on Tadalafil levels in your blood can be variable.
  • You should see your doctor if you feel faint, dizzy, or get a fast or irregular heartbeat, after taking the tablet.
  • Store below 25C in a cool, dry place, protected from light. Keep out of the sight and reach of children. Keep in the original packaging.
  • Do not use after the expiration date shown on the packaging.

It is a prescription combined tablet that helps to manage ED (erectile dysfunction) and PE (premature ejaculation) among adult men. No, Tadalafil 20 mg + Dapoxetine 60 mg is an on-demand tablet and not a daily dosage medication and must not be taken more than once in a 24-hour period. There are different Tadalafil tablets available for daily management of ED; consult your doctor.

Tadalafil can lower blood pressure significantly when taken in combination with antihypertensives or alpha-blockers. Let your doctor know before taking Tadish-D if you are on any of the blood pressure medications. Nitrates (e.g., nitroglycerin) are absolutely contraindicated with tadalafil.

Do not drink alcohol with this medication, as its combination with dapoxetine strongly promotes fainting, dizziness, and reflex tachycardia, so that even moderate amounts of alcohol may impair the tablet's effect.

Men suffering from unstable angina or those with recent (in the last 3 months) myocardial infarction or heart failure or those taking nitrate treatment should not take this medication. Men suffering from stable, controlled cardiac conditions should take the medication only after discussion with their cardiologist.

Tadalafil (in Tadish-D) is used over a much longer time period due to its significantly longer half-life (~17.5 hours compared to that of Sildenafil, which is about 4 hours), and hence it is frequently referred to as the "weekend pill." Both are PDE-5 inhibitors but differ in how long they last and their effects with relation to food.

Contact your doctor or go to the nearest hospital for immediate medical help if you have an erection lasting longer than 4 hours (priapism), sudden decrease in vision in one or both eyes, sudden decrease or loss of hearing, severe chest pain, irregular heartbeat, or fainting. These effects are rare but serious.
